Families often need just a little structured support to prevent a much larger crisis.

Our Caregiver Stabilization & In-Home Support Fund

This fund provides short-term, non-medical in-home support in structured 2-hour increments (AM or PM visits) for families in urgent need.

We do not provide 24/7 care.
We do not provide skilled nursing or medical services.

Instead, we coordinate focused support visits designed to stabilize the home environment.

During each scheduled visit, support may include:

✔ Light cleaning and trash removal
✔ Meal preparation and grocery organization
✔ Medication reminder oversight and compliance checks
✔ Putting away groceries and organizing supplies
✔ Standby assistance with personal hygiene
✔ Transportation coordination for specialist appointments
✔ Collaboration with providers and the care team (with consent)

These visits are designed to reduce caregiver overwhelm while we work alongside the family to secure long-term care resources.

This Is a Bridge — Not Long-Term Care

Our program offers a 1–4 week stabilization period.

During this time, we:

• Coordinate short-term respite relief
• Assist with benefits navigation (Medicaid, waiver programs, community resources)
• Help establish sustainable care plans
• Prevent unnecessary rehospitalization or emergency placement

The goal is stability — not dependency.
