I am working to launch a Medicaid Transition Case Management program through Fundamental Chromatic, focused on helping individuals safely move from institutions, unstable housing, or crisis situations into community-based living with support, dignity, and care.

This work already exists in practice—but to make it sustainable and reimbursable, I must first be authorized as a Medicaid provider. That authorization requires hundreds of hours of unpaid labor before any revenue is possible.

To even apply, I must complete extensive paperwork, policies, and procedures; meet strict compliance standards; complete required trainings; secure professional liability insurance; and establish HIPAA-compliant systems to protect client information. All of this must be in place before approval is granted.

At the start, this program will likely be staffed by me alone, serving as both founder and case manager. This fundraiser exists to support the time, tools, and infrastructure required to do this work ethically, legally, and responsibly.

Funds raised will help cover:

• Professional liability (O&E) insurance

• Required trainings, certifications, and authorizations

• A reliable, secure laptop with sufficient storage

• A dependable, non-corrupting backup hard drive

• HIPAA-compliant software, secure data storage, and cybersecurity

• Administrative, legal, and accounting support

• Transportation and communication costs for community-based work

• The unpaid labor required to complete the Medicaid authorization process

Approval to bill Medicaid is not guaranteed. However, this effort represents a real opportunity—a door and a window of hope—to transform unpaid care work into sustainable, reimbursable services that can grow over time.

Even while authorization is pending, your support strengthens Fundamental Chromatic’s capacity to serve the community with integrity, accountability, and compassion.
