On November 20th, 2025, the Hornbuckle Foundation's – Connections of Hope Outreach Team will host Sleep Out, a community event where we will be joining our community members on the street to raise awareness and support for those experiencing homelessness. 

This isn’t just a fundraiser—it’s an act of solidarity. Each night, hundreds of individuals in our own community face the harsh reality of sleeping without shelter. For many, access to basic needs like hygiene supplies, warm clothing, and even a listening ear feels out of reach. These aren’t just statistics—they’re people with stories, struggles, and strength. 

Through the Sleep Out, we aim to: Raise $10,000 to support outreach recovery efforts, collect hygiene supplies (soap, toothbrushes, feminine products, etc.) and gather warm clothing (coats, gloves, socks, hats) ahead of the winter season. 

We will document the experience of navigating systems of support while unhoused as well as gathering insight from those we serve—shedding light on the real barriers people face when trying to receive help. We believe that empathy grows through experience. That’s why we’re choosing to sleep out, side-by-side with the people we serve and provide a listening ear to those who feel unseen. We will step into the uncomfortable and vulnerable spaces too many call their everyday reality. 

But we can’t do this alone.

Because no one should have to face the cold—and the system—alone. 

"Connections of Hope" is a peer outreach program supported by the Region 9 Opioid Settlement Funds Council. It is a comprehensive outreach program dedicated to transforming the lives of unhoused individuals by providing a pathway to recovery and stability. Our mission is to offer accessible, professional services including detoxification programs, treatment facilities, sober living environments, secure housing, and thorough peer case management. We believe that the foundation of lasting change lies in the power of empathy and shared experiences; hence, our work is significantly facilitated and enriched by peer support specialists. These specialists, who have triumphantly navigated their own journeys to recovery, bring invaluable insights and empathy, fostering an environment of trust, motivation, and hope. Through collaboration, we not only address the immediate needs of the homeless community but also empower them to envision and achieve a future filled with dignity, health, and self-sufficiency.

Program Impact:

The Hornbuckle Foundation (2024–2025)

Over the past year, The Hornbuckle Foundation has remained steadfast in our mission to support individuals and families affected by addiction, providing life-changing resources that foster long-term recovery, housing stability, and economic independence.

From October 2024 through September 2025, we proudly served 2,426 individuals in our community. Through personalized support, recovery services, and partnerships with local organizations, we helped our participants build stronger foundations for their future.

  • 226 individuals gained stable housing, creating safer and more secure environments for themselves and their families.

  • 388 individuals secured employment, empowering them to achieve financial independence and stability.

  • 142 participants actively engaged in recovery, taking courageous steps toward long-term healing.

  • 134 new individuals started services, marking the beginning of their recovery and transformation journey.

These outcomes reflect not only the strength and resilience of those we serve but also the power of community support and comprehensive recovery services.

Every person we serve represents a story of hope, progress, and potential.

Together with our partners, donors, and volunteers, we continue to restore lives, strengthen families, and build healthier communities- one connection at a time.
