The CaringConnect Program at Rocky Mountain Crisis Partners (RMCP) is a supportive care initiative that provides follow-up services for individuals discharged from hospitals, substance use treatment programs and legal settings. CaringConnect delivers relational, trauma-informed care designed to bridge critical gaps in the behavioral health system and ensure continuity of support during periods of elevated risk-offering connection when it matters most.
