Northwest Colorado Community Health Partnership, Inc.

Why Give to The Health Partnership?

The Health Partnership ensures all residents in the Yampa Valley have access to primary care & behavioral health services, recovery support services, housing, transportation & nutrition. We connect the dots between people, providers, and community resources, working alongside local partners to fill gaps, reduce barriers, and prevent duplicative efforts. By strengthening the network of support across our communities, we help ensure that no one falls through the cracks and that every person has the opportunity to live a healthier, more connected life. All of our services are completely FREE with no barriers to access.

None of this happens alone. It takes dedicated people, strong partnerships, and the resources to turn good ideas into lasting solutions.

Your donation helps us:

  • Strengthen partnerships between local organizations to create a more seamless system of care
  • Identify community needs and lead collaborative solutions that improve health and well-being for everyone

Testimonials

"Brook has been such an amazing part of my new chapter of life. I started my real journey of recovery at the Foundry in Steamboat. After deciding to stay in this area Brook has been more than a great peer counselor that has helped me help myself with such things as housing funding which made it possible for me to thrive instead of sink, auto parts so me and my family have a reliable vehicle, and childcare in which has enriched both mine and my sons' lives. Also, the regular check ins I always look forward to because of her insight and warm heart. When I moved to Craig, she is and was part of the reason I feel at home here. The transition period had the potential to be overwhelming. But instead, it has been the beginning of the best part of my life. Brook is an angel and I am blessed to know her and the health partnership is more than lucky to have her." - THP Client

"This quarter, I worked with a client who had nowhere else to go. They had just been diagnosed with a critical illness, could no longer work, and had no direction in how to move forward. Through a referral from a trusted community partner, I was able to meet with this client and develop a path forward together. Since barriers often present when one tries to navigate resources alone, I was able to guide this client through the process of applying for FAMLI leave so they could properly rest and take care of themself while easing the financial burden of taking leave from work. While we were waiting to hear from FAMLI regarding their benefits we were also able to cover one month of rent to further reduce the stress on the client and their family, and further their stability during this tumultuous time. I am thrilled to say that my client is fully recovered, back to their job, and thriving. My client consistently told me they don’t know what they would have done without our community resources. These programs are lifechanging and help build a robust and resilient community. Trust and collaboration have been a cornerstone of my direct client work and community partnerships to make outcomes like this possible. My client is endlessly grateful to live in the Yampa Valley, where organizations collaborate to build a thriving community." - THP Care Coordinator

“The CO Rural Peer Recovery Conference is honestly my favorite conference every year. I always leave feeling inspired, with new ideas, new connections, and practical tools I can use in my work. The speakers were amazing, and I appreciate all the hard work that goes into making this conference such a meaningful experience. I'm already looking forward to next year!” - Peer Recovery Conference Attendee

Background Statement

Our History

The Health Partnership began with a simple but powerful idea: our communities are stronger when we work together. In 2006, healthcare providers across Northwest Colorado came together around a shared agenda to address the health needs of the Yampa Valley and build stronger, more resilient systems of care. That spirit of collaboration continued to grow, and, in 2016, evolved into the nonprofit organization is it today, Northwest Colorado Community Health Partnership.

Over the past 10 years, The Health Partnership has grown alongside the communities we serve. We are proud of the partnerships we have built with local businesses, healthcare providers, nonprofits, government agencies, and community organizations, and even more proud of the people we have had the privilege of walking alongside through some of life’s most challenging moments. Every partnership, every connection, and every person we have supported has helped shape who we are today.

As a 501(c)(3) nonprofit, The Health Partnership is funded primarily through federal and state grants. At our core, we are guided by a deep commitment to advancing health equity and strive to foster diversity, equity, and inclusion in everything we do. As we look toward the next decade, we remain committed to strengthening the connections that cultivate a thriving community.

Our Journey Through the Years

2017

Launch of the Care Coordination program, providing one-on-one support to help people access transportation, primary care providers, housing & financial stability, transportation and other needed resources. We also launched the Navigation Networks, bringing together case managers, navigators, and care coordinators to identify available resources and create county-specific resource and referral lists in English and Spanish.

2018

Navigation Networks evolves into Collaborative Care meetings to support wraparound services and improve coordination among direct-service staff.

2019

Northwest Colorado Community Health Partnership is renamed The Health Partnership. Health insurance outreach and enrollment services begin, helping people find plans and understand how to use coverage through the health exchange.

2020

Launch of the Community Recovery Program in response to the need for substance use disorder recovery support and safe, substance-free spaces. Also efforts to bring the Community Resource Network online to support virtual care team collaboration meetings.

2021

Programs are consolidated and strengthened, High Fidelity Wrap Around Services is introduced for youth with complex mental health needs, and a two-year strategic plan is drafted.

2022

Completion of the Community Health Needs Assessment; launch of Jail Care Coordination in Routt County; and reestablishment of Care Coordination Collaborative meetings in Routt and Moffat counties.

2023

Launch of the Recovery Friendly Workplace Initiative (RFWI) to help organizations and businesses support staff in substance use disorder recovery.

2025

The organization reaches its largest scale yet, with six Care Coordinators serving Routt, Moffat, and Rio Blanco counties; the Peer Recovery team becoming the largest peer recovery program in Northwest Colorado; the Judicial Program adding dedicated management and launching the Handle with Care initiative with the Moffat County Sheriff's Office and the Moffat County School District; Health Coverage being recognized by Connect for Health CO as a model site; and work beginning on a Social Health Information Exchange (SHIE) to streamline referrals.

Looking forward, The Health Partnership remains dedicated to expanding its reach and impact, steadfast in its mission to advance health equity and foster a healthier, more connected community. Together with its partners and community members, it continues to build a future where everyone has access to the care and support they need.
