Background Statement
Sunrise Community Health is a non-profit, award-winning federally qualified health center serving Northern Colorado from 15 clinic sites in Greeley, Evans, Windsor, and Loveland. Our dedicated and talented team of over 450 offers medical, dental, and behavioral health care as well as laboratory, X-ray, pharmacy, social work, health education, WIC, and care management services to 45,000 people each year. Also, Sunrise operates the Immigrant and Refugee Program of Northern Colorado (IRCNoCo), a program that empowers over 2,000 refugees and immigrants, connects communities, and advocates for successful integration.
As a Community and Migrant Health Center, Sunrise serves primarily low-income, uninsured, and underinsured individuals and families; 9 out of 10 Sunrise patients live at or below 200% of the Federal Poverty Level, 60% have public insurance, and 1 in 4 are uninsured. Sunrise also serves a diverse patient population - with nearly 70% from racial and ethnic minorities and four out of ten who are best served in a language other than English.
Sunrise is dedicated to developing the next generation of health care workforce. As a recognized leader in education health center activities, we offer 20,000 hours of education each year to students of all disciplines - physicians, dentists, physician assistants, nurses and nurse practitioners, medical assistants, social workers, and high school students who are just beginning to explore healthcare as a possible career.
Sunrise strategically designs community collaborations to connect resources and care to serve the whole person in a coordinated, efficient, and economical manner. Our innovative partnerships reach across the spectrum of care to include behavioral health, public health, hospital systems, specialists, and homeless services, as well as community-based care navigation and care management.
As a Federally Qualified Health Center (FQHC), we provide services regardless of a patient's ability to pay by screening patients for eligibility for public funded programs or private insurance and by offering a sliding fee scale and payment plans.
