Background Statement
Elevated Community Health was established in 1993 as a weekly walk-in clinic staffed entirely by volunteers and managed by Summit County Public Health. The Care Clinic was founded to address the need for primary and preventative health care for people who are uninsured and have low incomes. It became a 501(c)(3) in 2005 and gained designation as a Federally Qualified Health Center in 2015. In September 2022, the Care Clinic opened Mountain Cares Pharmacy in the Frisco Medical office, creating improved access to low-cost medications for Care Clinic patients. The Care Clinic serves nearly 9,000 patients annually with integrated medical, dental, and behavioral health services. Service locations include the main medical and dental clinic in Frisco (Summit County), seven School Based Health Centers located in Summit, Lake, and Park Counties, and Lake County Dental in Leadville (Lake County).
