Located in the very southwestern corner of Colorado, Southwest Health System (SHS) meets the health care needs of a diverse population of low and middle income families, retirees, and Native Americans from the Ute Mountain Ute tribe. In most years, patient fees do not cover expenses for SHS. The Foundation Board, made up of community members and health care providers, solicits donations from individuals and local business and awards grants to SHS for needed equipment, based upon priorities established by SHS.

Over the last several years the Foundation has awarded over $75,000 in Cancer Care Grants to individuals living in Montezuma County. The $1,000 grants can be used to assist in the everyday needs for cancer patients and their families. Donations to the Foundation may also assist in purchasing necessary medical equipment. Donor support in 2024 and 2025 enabled the SWMH Foundation to purchase the following equipment for SHS: Emergency Medical Services (EMS) Ambulance Equipment, Obstetric Ultrasound for the Mancos Valley Health Clinic, Jaundice meter for Southwest Medical Group, a Pulmonary Function Testing Machine, and Fiberoptic Endoscope for the hospital.
