South Routt Medical Foundation: A Community That Cares

At the South Routt Medical Center, we believe that healthcare is more than just a service—it’s a shared commitment. Since our founding in the early 1960s, our community has stood beside us, helping us grow from a small rural clinic into a trusted hub for compassionate, comprehensive care.

In rural communities like ours, access to quality healthcare is not just important—it’s essential. With long distances to larger hospitals and limited local resources, rural healthcare centers serve as lifelines. We provide critical services close to home, ensuring that families, seniors, and individuals of all ages can get the care they need without delay or travel hardship.

Today, we offer a wide range of services—from family medicine and pediatric care to women’s health, urgent care, and in-house diagnostics. But what truly defines us is the heart behind the care: a team of dedicated providers and staff who treat every patient like family.

Nearly 50% of our patients rely on Medicare or Medicaid, making our services not just convenient—but absolutely vital. Your support helps ensure that everyone in our community, regardless of income or insurance status, has access to high-quality, compassionate care.

We are proud to be part of a community that shows up—volunteering, fundraising, serving on boards, and supporting our mission in countless ways. Your generosity helps us expand our services, improve our facilities, and reach more people with the care they deserve.

When you give to the South Routt Medical Foundation, you’re not just supporting a clinic—you’re investing in your neighbors, your family, and your future. Together, we’re proving that rural doesn’t mean forgotten. It means resilient, connected, and proud.

Thank you for being part of a community that cares—and for helping us keep rural healthcare strong.

Background Statement

Nestled in the heart of Oak Creek, Colorado, the South Routt Medical Center has long stood as a beacon of health and wellness for the Yampa Valley. Established in the early 1960s—its original structure arriving by rail car—the center reflects the community’s deep commitment to accessible, high-quality healthcare.

Over the decades, South Routt residents have consistently rallied behind the center, supporting its growth into a trusted provider of comprehensive care for all ages. From infants to older adults, the center offers a full spectrum of services, including pediatric and adult primary care, women’s health, urgent care, and essential diagnostics such as EKGs, ultrasounds, and lab testing.

In 1992, the center became a 501(c)(3) nonprofit, and by 1996, the South Routt Medical Health Service District was formed to further strengthen its mission. Nearly 50% of patients rely on Medicare or Medicaid, underscoring the center’s vital role in ensuring care for all, regardless of income or insurance status.

The center is deeply woven into the fabric of the community, offering health education programs, elderly footcare, and partnerships with local schools. To meet growing demand, the center is planning a 1,500-square-foot expansion—adding exam rooms, office space, and an upgraded lab. The project will also include a whole-building cooling system, a backup generator, and a paved parking lot to improve safety and accessibility. This expansion is expected to create jobs, support local businesses, and contribute to Oak Creek’s broader medical tourism strategy.

The South Routt Medical Center remains steadfast in its mission: to provide compassionate, community-centered care and to adapt to the evolving healthcare needs of the region. More than just a clinic, it is a vital part of South Routt’s identity—committed to improving lives and building a healthier future for all.
