Today, we offer a wide range of services—from family medicine and pediatric care to women’s health, urgent care, and in-house diagnostics. But what truly defines us is the heart behind the care: a team of dedicated providers and staff who treat every patient like family.

Nearly 50% of our patients rely on Medicare or Medicaid, making our services not just convenient—but absolutely vital. Your support helps ensure that everyone in our community, regardless of income or insurance status, has access to high-quality, compassionate care.

Mission

The South Routt Medical Foundation exists to ensure that every resident of South Routt County has access to high quality, compassionate healthcare. We believe that care should be available to all, regardless of income, age, or insurance status.

We proudly support the South Routt Medical Center in its mission to deliver essential medical services close to home. In rural communities, healthcare is not a convenience—it is a lifeline. Through fundraising, community involvement, and strategic investment, we help sustain and expand the care our neighbors depend on.

Driven by the belief that rural communities deserve the same level of care and attention as urban ones, we are committed to building a healthier, more resilient South Routt. Our work strengthens access to care across Oak Creek, Yampa, Phippsburg, Stagecoach, Toponas, and the surrounding unincorporated areas, ensuring that no one is left behind.

The South Routt Medical Foundation was created to serve as the nonprofit fundraising arm of the South Routt Medical Center, helping to secure the resources needed to grow and adapt to the evolving needs of our region.

Background Statement

Located in the heart of Oak Creek, Colorado, the South Routt Medical Center has been a trusted source of health and wellness for the Yampa Valley since the early 1960s. Its original structure arrived by rail car, a symbol of the community’s determination to bring healthcare within reach.

Over the years, residents from across South Routt have rallied behind the center, helping it grow into a comprehensive provider of care for individuals and families of all ages. Today, the center offers a full range of services including pediatric and adult primary care, women’s health, urgent care, and vital diagnostics such as EKGs, ultrasounds, and lab testing.

In 1992, the center became a nonprofit organization, and by 1996, the South Routt Medical Health Service District was formed to further strengthen its mission. Nearly half of our patients rely on Medicare or Medicaid, highlighting the center’s critical role in ensuring that care is accessible to everyone.

The center is deeply embedded in the life of the community. It offers health education programs, elderly footcare, and partners with local schools to promote wellness. To meet growing demand, the center is preparing for a 1500 square foot expansion that will include new exam rooms, office space, and an upgraded lab. The project will also introduce a building-wide cooling system, a backup generator, and a paved parking lot to improve safety and accessibility.

This expansion will not only enhance patient care but also create jobs, support local businesses, and contribute to Oak Creek’s broader vision for medical tourism. The South Routt Medical Center remains committed to providing compassionate, community-centered care and to evolving with the healthcare needs of our region.

More than just a clinic, it is a cornerstone of South Routt’s identity—a place where lives are improved, futures are strengthened, and the promise of rural healthcare is fulfilled.
