Background Statement

PrimeHealth+ is a Community Health Center, providing integrated primary medical, dental and behavioral health care to over 16,000 patients across seven counties in West-central Colorado. Ours is a unique health care facility due to the integration of multiple services under one organization, coupled with the offer of a sliding fee scale. These elements combine to offer an accessible, affordable and successful model of health care for persons struggling financially.

Since opening its doors in 1988 as a safety net health clinic that served only those without insurance, PrimeHealth+ has responded to the troubling gaps, barriers and disparities that prevent people from receiving health care: affordability and accessibility. Our ability to do this was enhanced and expanded when we became a Federally Qualified Health Center in 2015, allowing us to extend service to insured as well as uninsured residents.

PrimeHealth+ provides cares for patients at six permanent locations, making it easier for patients to access care. In Mesa County, all services are offered at our newest clinic at 526 29 1/2 Road and 2139 N. 12th Street, and in Montrose at 87 Merchant Drive. Two school-based health centers in Grand Junction offer primary dental, behavioral, and medical care to students and staff. Also in Mesa County, one dental hygienist is embedded in a local pediatric medical facility, and acute medical and dental screenings are offered each week for persons who are unhoused at the Catholic Outreach Day Center.

Speaking of our new clinic: on September 27, 2025, we opened the doors of a newly constructed health center in Grand Junction, located closer to nearly 50% of our existing patients on a campus shared with Mesa County Health & Human Services and the Workforce Center. At 27,000 SF, this building offers all services as well as a drive-up pharmacy and Convenient Care Clinic. With expanded hours, services, and capacity, we anticipate caring for up to 20,000 patients in this location within a few years.

PrimeHealth+ is the trusted healthcare home for thousands in our community. As a private, non-profit organization supported by donors, foundations, government health agencies, corporate and community partnerships, and patient revenue, PrimeHealth+ has been enabled and honored to have served hundreds of thousands of individuals over 37 years--including migrant workers/families, persons who are homeless, seniors/elderly, people between jobs and families striving to make ends meet on modest incomes.
