We are a community care hub that bridges gaps between healthcare, social support services, and the community. We do this by improving access to and navigation of resources. We improve community health through resident-driven insights, data-informed action, and policy and workforce development.
From day one, Highroots has been committed to honoring lived experience and connecting those voices with data, research, and resources to build meaningful solutions.

Here’s a snapshot of our impact so far:
• We've concluded community assessments that have helped secure over $270,000 in funding for our partners.
• We’ve engaged hundreds across Colorado through wellness consulting and health education programs.
• We’ve partnered with over ten organizations and leveraged research to shape programs that are grounded in equity and inclusion.
• We’ve launched a Youth Health Ambassador program in partnership with Peak Ed and the Windy Pines Foundation and initiated scholarships for aspiring community health workers.

Our vision is to see our neighbors thrive in a transparent and supportive environment, leading in decisions that affect their economic stability, education, healthcare, neighborhoods, and social support systems.

Our organization was founded on the countless stories that individuals from the community have shared with us. These stories often reflect the challenges of everyday life that snowball into health issues with continued barriers to accessing resources.
Taking a preventative approach means listening deeply to the specific experiences of people in their communities as they live their lives. It means identifying the short-term challenges people are experiencing before they turn into long-term catastrophes and providing solutions that are led by our neighbors.
We achieve this goal by conducting community health needs assessments and evaluations, co-creating health programs, and providing training and support to community health workers.
