Mission Statement

Improve quality of life for all Northwest Colorado residents by providing comprehensive health resources and creating an environment that supports community wellness.

Our values:

Access to health is everyone's right
Regardless of their income, race, age, gender, health status, or insurance coverage, everyone deserves the chance to be healthy and well. No one should be denied the opportunity to see a doctor, receive healthcare, or participate in preventive health programs.

We focus on health, not just healthcare
Health and wellbeing are at the heart of everything we do at Northwest Colorado Health. Through our proactive, preventive programs and services, we strive to help people stay healthy and well. While we do treat illness and injury, our goal is for every member of our community to be empowered with the information, tools, and resources they need to live a healthy life.

Compassionate, personalized care improves outcomes
How we deliver healthcare makes a difference to our patients and their health results. We treat everyone with compassion, integrity, and the utmost respect. We take the time to really understand every patient's situation and never pass judgment. By meeting patients where they are with kindness and empathy, we serve as their partners on their health journey.

We deliver only the highest quality programs and services
We take great pride in providing the most advanced and effective programs and services. We hold ourselves to the highest standards to ensure our patients receive exceptional care. As a non-profit organization, we always focus on providing what is best for each patient rather than what makes the most money.

We lead by example
Every day, we strive to model the healthy behaviors we promote to our patients. From eating well and exercising to preventative screenings and regular check-ups, we practice what we preach.

Organization History

At Northwest Colorado Health (formerly Northwest Colorado Visiting Nurse Association), we believe that everyone deserves the chance to achieve their best health. Northwest Colorado Health was founded in Steamboat Springs, Colorado in 1964. What started out as a small agency providing only public health services and home health care has grown to also provide primary care, hospice and palliative care, and wellness and prevention services to all members of our community. With our programs, our facilities have grown, as well.

2008 Opened the Community Health Center in Moffat County
2009 Opened the Rollingstone Respite House in Routt County

Since our inception more than 50 years ago, Northwest Colorado Health has embraced continuous performance improvement in order to provide all residents of Northwest Colorado with the topnotch, timely health care services that they need. Northwest Colorado Health continues to innovate and keep pace with healthcare best practices and national health care reform, such as receiving Patient Centered Medical Home recognition, spearheading a regional integrated behavioral health program, and expanding insurance enrollment services to reach four counties in Northwest Colorado.

Northwest Colorado Health provides health programming across the region to people of all ages, cultures, income levels and life situations. All services are offered on a sliding scale; Northwest Colorado Health gives all residents of Northwest Colorado a chance to live a healthy life through five major program areas: Prevention Services, Home Health, Hospice and Palliative Care, Primary Care, and The Haven Assisted Living.

Testimonials

"I kept losing more and more teeth until they were pretty much all gone. I started avoiding people because I was embarrassed. I wanted to fix my teeth, but I couldn't afford it. Thankfully, my boss helped me get set up with Northwest Colorado Health and it was wonderful. I got the rest of my teeth pulled and dentures. The first time I looked in the mirror with them on, tears came to my eyes. Now, I don't mind talking to people, and even my overall health is better. Northwest Colorado Health took such great care of me and I am proud of my smile." - Chuck Thompson
