Description

For clients who live in their own homes or with family members (basically providing all of the same services as for Residential Comprehensive Services and including much of the Vocational/Day Programming).

"George" was exploited financially as a youngster and was consequently fearful of making money, reporting wages, and paying taxes. He didn't want to "get in trouble" again. But "George" always wanted a job. "George" likes to learn new skills and is very conscientious. "George" joined Six Points a little over a year ago and was first about meeting staff and other clients. Gradually, "George" began to trust his Six Points' family and joined in art, athletic games, and social activities. "George" quickly became successful at these activities. His confidence grew. Eventually, "George" shared his dream of getting a job. Six Points helped him complete employment applications, learn new job skills and become confident at interacting with store customers and staff. "George" is now a valued employee and is managing his own money. His self-esteem and communication skills have improved vastly and "George" is proud to be a valued member of our community.

Description

Providing job services and day programs for clients including:
•employment assessment and review;
•vocational training, specialized habilitation and one on one support in conducting job tasks;
•job placement assistance;
•serving as the liaison between outside employers and Six Points' clients; and
•supported community connections - getting the clients integrated into the community

"Frazier" had a challenging time keeping a job because of his disability. Through job placement assistance and vocational training he learned how to be a grocery store clerk and then promoted to a cashier and with constant practice has been promoted within the organization to another department. He has also learned how to save money through financial assistance from our staff. He has planned and taken many trips/vacations with the help of our staff. All of the staff are very proud of what he has done in such a short period.

Description

Residential Comprehensive Services and Client Training for seven clients who live in housing owned by Six Points, including:
• personal care - eating/drinking, personal hygiene, emergency response, medications, supervision, health monitoring, and positioning/transferring;
• household assistance - money management and disbursements, shopping assistance, laundry, meal preparation, household cleaning and maintenance;
• mentoring - decision making, planning, appointment scheduling, safety, health, dietary assessment, relationship education and support;
• transportation - accessing community services, training programs, employment, appointments, shopping, banking, civic events or social activities; and
• community accessibility/connections - acquisition of community assistance, recreational/community activities, adaptive skills, socialization, and supplies.

Evidence of Program's Success

One of our clients had serious family issues growing up and a history of abuse. "Suzy" is diagnosed with cognitive impairment, intermittent explosive disorder, and ADHD. Thanks to all of the monetary support from the community, we had staff spend more one on one time teaching "Suzy" how to control her temper and how to stay on task. After a year and a half with our organization and through our vocational training, "Suzy" is now very successfully employed and just received a promotion at a busy community business. When she comes into Six Points, you can just tell that her confidence has increased and that she has a very high level of self-esteem.
