Envision, Creative Support for People with Developmental Disabilities has served as a cornerstone in Weld County for the past 50 years. Our mission is to enhance the quality of life for people with intellectual and developmental disabilities. As a non-profit, our success is reliant upon the generous support of the local community. We value every donation and work diligently to maintain less than 10% administrative costs so that funds can be directed to services for the people we support.

Description

Envision provides case management for adults with intellectual and developmental disabilities in Weld County. This may include services through State Supported Living Services and Comprehensive Services waivers. Envision conducts eligibility determination screenings and manages an ongoing waiting list for Weld County.

An annual survey was completed to assess satisfaction of the program. Responses were received on March 1, 2017 and data collected is being used to improve case management services the people Envisions supports.

Description

Provides case management for children on the Medicaid Children's Extensive Services Waiver (birth until 18th birthday), Family Support Services Program, Children with Autism Waiver (birth until 6th birthday), and the Early Intervention Program which provides services for children (birth to 3 years of age) with developmental delays.

Annual surveys consistently show a general satisfaction of the program, and we are meeting all state and federal program requirements. Changes are constantly being made to increase satisfaction on services provided, and having the case manager offer more opportunities for families to access services and other community resources.

Description

Envision Program Services provides direct care to adults with intellectual and developmental disabilities in Weld County.
Services include Residential supports (host home providers, personal care alternatives), Day Program, (supported community connections, specialized habilitation services) and Supported Employment (Work Crews and Individual Community Employment).

Satisfaction surveys consistently show a general satisfaction of the program. Changes are constantly being made to increase satisfaction and offer more opportunities. Additionally, people supported make very positive comments frequently about the services they are receiving and express happiness in their words, actions/interactions and body language.
