West Mountain Regional Health Alliance Overview

West Mountain Regional Health Alliance brings together providers around food security, homelessness response and housing, and community connections. We have grown to now have two coalitions that are targeted towards the two biggest needs in our community- food and housing insecurity and then as well as linking people with resources through our community health connections. WMRHA works to serve all people who need support by improving the health system. We have assembled resources that help folks identify and access the most appropriate services in their journey to care for themselves, families, friends, and neighbors.














Programs

The Valley Alliance to End Homelessness (VAEH)

The Valley Alliance to End Homelessness (VAEH) is a collection of county, non-profit, and state-level partners invested in finding solutions to homelessness for our region. The VAEH is currently addressing homelessness in the tri-county area through supporting clients to access temporary and permanent supportive housing and advocating for better solutions in order to reach its goal: to make homelessness in our region rare, brief, and met with compassion.


The Mountain Coalition for Food and Nutrition Security (MCFNS)

The Mountain Coalition for Food and Nutrition Security (MCFNS) is a partnership with food security organizations and local governments in the tri-county area, working together to promote food security as a key determinant of health within the region. MCFNS believes that food and nutrition is a right that can best be delivered through a regional approach that provides sustainable food access, education, and storage to ensure the health of our region. 


Food Tote

WMRHA started a Food Tote Program in 2021 as a result of the Accountable Health Communities Model program identification as food as the highest need in our community. We currently have 5 clinics in the Valley apart of the food tote program: Roaring Fork Family Practice, Mountain Family Health Center, Aspen Valley Primary Care, Valley View Hospital and Mind Springs Health. We are providing these food tote bags to people who identify as "in need of food" by these clinics. The different bags we provide include culturally appropriate food, food that is senior appropriate, food for chronic health individuals, and non-cooking families. 















How our work supports our community

Here is a story of how our help and intervention helped... For those experiencing homelessness every day is survival. For a new mom with a baby, survival isn't enough. When Jose, a Homeless Street Outreach worker met this woman, she was about to have a baby, and he started the process to find her housing. Jose knew that she needed a place for her and the new baby right after the birth. Jose worked with West Mountain and Salvation Army to line up temporary shelter at a hotel. This temporary shelter made it possible to bridge the gap until housing was secured. This is just one example of collaboration that leads to helping those in need find a home to move from surviving to thriving.
