We are honored to be a beneficiary of this year's 2025 Service Academy Alumni Golf Tournament! Proceeds will benefit Home Front Military Network, helping connect Colorado’s service members, veterans, and their families with the vital resources they need.

About HFMN

Before there was Home Front Military Network, there were two passionate forces standing steadfast for those who serve: The Home Front Cares (THFC) and Peak Military Care Network (PMCN).

Founded in 2004, The Home Front Cares (THFC) provided emergency financial assistance to Colorado’s service members, veterans, and families, partnering statewide to ensure no one was left without support. In 2013, Peak Military Care Network (PMCN) united 47 partner agencies to connect individuals with employment, housing, behavioral health, and other resources, preventing crises through coordinated navigation and culturally informed care. In January 2020, THFC and PMCN merged to form Home Front Military Network (HFMN), streamlining access to services and amplifying impact. Today, HFMN’s case management team supports clients for as long as needed—addressing financial stability, housing, benefits, and follow-up care—with demand for long-term case management rising from 10% pre-merger to 88% in 2024 due to increasingly complex needs.

Thanks to your support, HFMN's five year impact includes:

  • HFMN handled more than 69,000 calls for assistance
  • A 400% increase in calls compared with our call volume pre-merger
  • Over 46,700 total referrals to HFMN since 2020
  • HFMN's partner network has served more than 300,000 clients throughout Colorado 
  • Since 2020, HFMN has provided $1,348,414 in emergency financial assistance, keeping over 990 households from becoming homeless or without other basic needs. Collectively we have provided more than $6.5 million in emergency financial assistance since 2004.  
  • 95% of all clients reported lower stress and anxiety levels following HFMN assistance
  • 96% of financial assistance clients remained stably housed 6 months after assistance; and 99% indicated decreased stress and anxiety levels.
