As a result of the generosity and support of donors like you, Sangre de Cristo Community Care has been able to provide the highest quality end-of-life care and supportive services to residents of southern Colorado for over 30 years regardless of their ability to pay. Sangre de Cristo Community Care serves 16 counties in southern Colorado.

Mission

Our mission at Sangre De Cristo Community Care is proven compassionate care to patients and families during rehabilitation, chronic and serious illness.

Background Statement

Sangre de Cristo Community Care was founded in 1985 as a non-profit hospice provider initially funded by the two primary hospitals in Pueblo – St. Mary-Corwin Medical Center and Parkview Medical Center. The call to care for those who are dying with the compassion exemplified in a hospice model was answered by committed volunteers who became the first staff and board members. In 2001, services expanded to include palliative care. In 2008, the Joni Fair Hospice House opened as the only inpatient facility in Southern Colorado dedicated to hospice care. Due to community needs and the closing of the only home health agency in Huerfano and Las Animas counties, Sangre expanded services to include home health care in 2019. To encompass all the programs that we now have, the decision was made to rebrand the organization and change our name to Sangre de Cristo Community Care in 2020. Today, Sangre de Cristo Community Care serves 16 counties throughout 23,000 square miles of Southern Colorado.
