Description

Awareness and Prevention focuses on educating the general public, those at highest risk for infection, healthcare providers, and nonprofit organizations about hepatitis C transmission, the benefits of early detection, healthy lifestyle changes, community resources, and more. Workshops, presentations, community dinners, health fairs, outreach events, and awareness events are held frequently throughout the year in a variety of locations. Public awareness campaigns feature public service announcements and media releases in print, via radio PSAs, through press releases, and on social media platforms.

Evidence of Program's Success

In 2019, we screened over 1200 individuals for hepatitis C, provided educational materials at 9 events, we delivered 73 educational presentations to 1505 individuals, and educated 7 community based organizations, prevention providers, social workers, and other health care providers in Colorado about viral hepatitis. Our Connections e-newsletter was distributed to over 1,300 email addresses and our Facebook page is followed by 1800 people.

Description

Our Patient Navigation program focuses on reaching those living with or affected by the hepatitis C virus or other liver diseases. Patient navigation helps to remove barriers to care and assist with an accurate diagnosis and timely treatment. Some of these barriers may include a lack of medical insurance, the cost of treatment, addiction, additional disabilities, or other health conditions or symptoms related to hepatitis C. Other resources include our toll-free confidential HelpLine, our monthly Connections e-newsletter, our online and social media presence, as well as printed educational materials that are provided to organizations throughout the state of Colorado.

Evidence of Program's Success

In 2017, we provided direct support to over 800 individuals on our HelpLine, Help-4-Hep's hotline, and Linkage to Care program. We assisted over 50 clients in entering HCV treatment and helped nearly 20 obtain cure.
