Background Statement
Originally founded by community leaders and volunteers in 1978, we were identified as Hospice of Weld County. During our inaugural year we served 18 hospice patients. The focus and vision of the hospice industry at that time was a grass roots movement to create community based programs to promote end of life care for patients with terminal illness.
At inception, the identified mission was to provide quality end of life care, console the grieving while educating the community of Weld County. Since that time, Hospice of Weld County recognized that services were needed in surrounding counties in the northern region of Colorado. In 2000, the name of the agency was changed to Hospice and Palliative Care of Northern Colorado in order to reflect the growing need of end of life care and present the palliative concept beyond the Weld county parameter.
In 1995 Hospice and Palliative Care of Northern Colorado opened our hospital based in-patient hospice unit (the In Patient Care Center) at North Colorado Medical Center in Greeley, Colorado. Our administrative offices moved into our current location at 2726 West 11th Street Road in Greeley in June, 1997. Since the later part of the 90's, we have expanded to serve patients in Larimer Counties.
