Colorado Gives Day benefits the HopeWest Dementia Support program!
A dementia diagnosis affects the entire family. As memory loss progresses and care needs increase, caregivers often find themselves feeling overwhelmed, exhausted, and unsure where to turn for help. The HopeWest Dementia Support program is here to provide education, guidance, support, and relief to caregivers.
Through the Medicare GUIDE model, HopeWest is able to provide coordinated dementia care and support designed to improve quality of life for both individuals living with dementia and their caregivers.
Our dedicated care navigators and medical providers walk alongside families, helping them navigate each stage of the dementia journey. These professionals step in to connect families to vital services, support groups, respite care and more.
Your support helps local families access services and resources that make the dementia journey less overwhelming and more manageable. Families will benefit from:
- Comprehensive and cognitive evaluations in partnership with the individual’s primary care provider help draw a better picture of their concerns and challenges at home.
- Monthly check-ins to see the patient, monitor disease progression, review care plans, and identify concerns before they become larger challenges for the family.
- Access to the HopeWest helpline whenever you have questions.
- Support from a care navigator to help to guide families and answer questions.
- Education and coaching to help you understand the disease and prepare for what is ahead.
- Respite care options so caregivers can rest knowing their loved one is safe.
Your gift helps families facing dementia receive the support, resources, and guidance they need. Plus, donations made through Colorado Gives Day receive a portion of the Colorado Gives Foundation Incentive Fund, increasing the impact of your donation.
We hope you'll support us this Colorado Gives Day!
