Description

Elk Run Assisted Living by Augustana Care provides assisted living and respite care to the people of Evergreen, Colorado. Amenities include the following:

* A beautiful mountain setting with walking trails and frequent sighting of wildlife.
* Therapy and Rehabilitation - A dedicated therapy center means residents of the community don't have to travel for therapy while recovering from an accident or illness. When they return home, outpatient services are also available.
* Beauty Salon & Barber Shop
* Medical Services - on site or through medical partners in the community.
* Great Room with Fireplace
* Library with Internet
* Life enrichment activities include scenic drives, a walking club, board and Wii games, concerts, crafts, exercise classes, volleyball, Bible study, discussion groups and more. Scheduled transportation is provided to area restaurants and shopping centers.
*Personalized care services provide those who need help with the activities of daily living, including setting up medications or getting help with dressing, bathing, walking or other needs. For residents who require temporary skilled nursing services, Elk Run partners with home health care agencies to provide the level of care each person needs in their chosen environment.
*Pet friendly

An estimated 25% of residents, who have been hard-working members of the community, will be able to live at Elk Run utilizing sources such as Medicaid and Medicare. Others will live at Elk Run for a few years, then find that they have encountered expensive issues they didn't expect, such as a long-term illness, or substantial increases in medication prices. For these elders, gifts will help to create a Bridge Fund, which has been used at other Augustana Care facilities to provide a "funding bridge" that keeps a resident in their apartment while the staff works with them to identify other potential sources of funding.

Colorado Gives Day funding will also support the transportation needs of the residents of Elk Run in Evergreen.

Augustana Care Elk Run residents, who have been hard-working members of the community, know that there is a caring staff that can help them through financial hardships that pop up as anxiety-producing surprises. Those who find that their funds are running out do not need to panic about their change in circumstance. They can be assured that the Bridge Fund is a resource to draw on, and staff will support them in identifying other potential sources of funding.

Description

Castle Peak located in Eagle, Colorado, opened in the Fall of 2016. It is the first senior care community in Eagle County, which is home to over 50,000 residents. The complex, which will allow older generations to stay in the community, includes the following:

20 assisted living rental apartments
22 skilled nursing private suites
12 memory care private suites
10 short-term rehab private suites

The Castle Peak facility, in Eagle, Colorado, features the following:
• Therapy and Rehabilitation - A dedicated therapy center means residents of the community don't have to travel for therapy while recovering from an accident or illness. When they return home, outpatient services are also available.

Castle Peak also features a promenade and dining terrace, courtyard, healing garden, fitness patio, community garden, a covered front entryway, and an expansive lawn.

Evidence of Program's Success

Castle Peak offers many opportunities to enjoy the amenities at the facility, as well as the best that the region has to offer. Funding will support on-campus activities such as music and horticulture therapy, which help residents to stay fit, socialize with others and continue to learn and contribute to the community.

Sixty-seven jobs were created in the first year of operation. Medicare/Medicaid certifications were received in the second quarter of the year and sixty individuals received short-term rehabilitation at Castle Peak and returned home.
