Description

ACC, in partnership with the Colorado Alliance for Health Equity and Practice (CAHEP) is launching an on-site medical clinic to assist low-income north Aurora residents in accessing free and culturally appropriate medical services. The clinic wil work in tandom with ACC's existing exercise and nutrition programs to create wrap-around preventative health care.

Description

People come in every day asking for information about resources in the community, such as those related to Emergency Food, Housing, Legal Services, and Adult Education. We work to provide the most appropriate referral and then follow-up at a later point to be sure they were able to successfully access the resources they needed.

Many families that have come to us for assistance refer other families and then stay to donate their time and talent as volunteers to help those families.

Youth Programs

Description

- Tutoring for Children ages 5-10:
Twice a week, for sessions of 1 hour, children receive tutoring to support literacy and math development. The target children are those who are low-income and at least one grade level behind in school.



- Youth Community Service Project:
High school youth have offered their time and skills to volunteer as part of ACC programs, such as tutoring during the summer program, tutoring in our up-coming academic support program for Elementary aged children and helping with our soccer program. We organize these youth in a way that provides them an opportunity to be of service at the same time they can receive support and guidance from ACC staff.

These are among our most popular programs and are consistently full and our forms of measurement show an increase toward all of our goals, such as increased literacy, increased physical activity and decreased risk factors related to disease and obesity.

Health Programs

Description

- Nutrition and Cooking class:
Working with Cooking Matters, we offer multiple 6-week classes focused on healthy eating and cooking and how to purchase healthy food on a budget.

- Medicaid and CHP+ Application assistance:
People come in on a daily basis seeking ACC support to apply for public health insurance, such as Medicaid or CHP+. We are proud that so many people trust us and seek our support, knowing that more and more vulnerable children will now have the support of health insurance.

- Resource and Referral to Medical Services (general):
People also seek out ACC help to access affordable medical care, including physical health, eye care, dental care and mental health services. We work with people to make appropriate referrals and follow-up to ensure that they successfully accessed the health care they needed.

- Exercise & Nutrition classes for adults and children:
We offer various classes for adults and children intended to provide opportunities for exercise, in combination with nutrition education. We offer Zumba and soccer for adults and a soccer program for children, three times a week for 1- 1.5 hours each session.

- Connect for Health program working with Aurora Coverage Access Network (ACAN). Health navigators assist families into Connect for Health Colorado as well as Medicaid (Health First Colorado). This is a critical service for our families. ACC has been a leader in Colorado health care enrollments.

Parenting Support

Description

- Nurturing Parents Classes:
This is a 8- 12 week, evidence based parenting class, offered onceweek for 2 hours each session. All participants take a pre- and a post evaluation in order to measure the impact of the program.

- Parenting Support Group:
A licensed facilitator guides the parent support group, 90 minutes each week.

Description

We offer three different English as a second language (ESL) classes 2 hours a week for 16 weeks. Citizenship classes, 2 hours/week for 10 weeks/session. During the summer, we offer an English conversation class, with a language exchange component (Spanish-English).

ACC also offers the Family leadership Training institute class. Students develop skills to become leaders in the community. 20 week program, once a week
